Eliminate Graininess and/or Blemishes after Shadow\Hilite?!?

Then I lightened it with Shadow\Highlite:

Now I notice the photo has some weird texture when enlarged. I don't think it's grainey, but I don't know how to describe it.

In CS2, I have tried Despeckle, Dust & Scratches, Reduce Noise, Guasian and Surface Blur.

Any other advice?

Instead of using shadow/highlight, apply adjustment layers using Levels, Curves and Color Balance
