first i think that real picture , i maybe wrong but it look real...
now you can simulate that i belive but it take a few layers and maybe blending mode..
just an idea of the workflow.... and other popel will have other ideas on the approach ...... remember there a lot of different way to achieve the same results..
1. you have your hands...
2. then fill white layer ..... erase all excess and just leave enough that covers the palm and wrists and arm.....
3. then you have a blue fil ... that cover part of the palm... probably use scrunge brush and a soft area brush for the edges and a soft area brush varying the opacity so that you can get that perceptions of the white background blending thru some of the blue...
4. and then you have a couple layers for that central design....
5. and of course the characters..... well if you can find a nice sloppy font... use that... either that or you try using the brush to manually draw in the characters on the fingertips....
now not sure if you need to use any blending modes although on most of the layers you might changing the layer opacity especially to retain the palm lines...
