Image to site?

Hi all!

Firstly, I am a noob and I am not aware of photoshop potentials, so I will just try to explain what I need...
I need to draw a table like that on a website.
That table must have some fileds in which i can write on. Specifically, this is about a dynamic form which will have fields like names, surnames, etc.
Is it possible to draw the table on photoshop and export it in a format that will allow me to do that?

There is a cool little program called Formtool that I used to use. It is really cheap and should do what you ask. Here is link to an older version.
