Adding Smoke Effects

Hello. I've searched around but I didn't see anything like this (here, or on another site).

I want to put a cloud of smoke in a photo so that it appears the person is exhaling. I've tried rendering clouds (most of the tutorials for smoke say to do this), but it isn't coming out the way I'd like.

The best example of what I want to achieve is probably this picture:

I'd also like to get the bloodshot and stressed eyes, if anyone knows how to do something like that.

Thanks for the help. I am not very efficient with Photoshop yet, so I'm sorry if this is basic stuff.

Try this:

-Create a new document with a black background (I used 150x150)
-Take the Brush tool, set the color to white and draw some lines (they don't have to be straight)
-Go to Filter > Blur > Radial Blur; use the default settings and just keep blurring until the lines are more smeer looking than line looking
-Go to Filter > Distort > Wave; use the default settings and keep hitting Random until you get what you like
-Duplicate the layer
-Set the Blending Mode of the new layer to Screen
-Go to Filter > Distort > Wave and do the same thing as the previous layer
-Keep repeating until you have a smokey looking effect

Nice. I like how this is turning out. I haven't gotten it quite the way I want it, but that's mostly because I'm still experimenting.
