Using Transform is duplicating image

Hi Guys, hopefully someone can help!

I have an image in a photoshop document, and whenever I click on the transform tool, it transforms the image, but it also duplicates it! It just all of a sudden started happening to everything I try to transform.

Also, if i even nudge the image (with the arrow keys) it creates a duplicate each time i nudge it!!

Please help me!! It's really annoying!!!

I know that happens if you hold the alt key while transforming / moving a layer so that is my first guess. If nothing else works you could always try resetting the Photoshop preferences.
