Use the Marquee tool and make a selection around what you would like to focus on. You can leave it like this, or go to Select-Transform selection, and turn it a bit in either direction.
Apply the transformation and copy it to a new layer, ctrl+J.
Right click the new layer - Blending options. Add a drop shadow, stroke, and a bevel if you wish. OK.
Make the original image black and white by hitting ctrl+shift+u.
Add some noise from Filter-Noise-Add Noise, to make the selection stand out a little more.
