Phase 1:
Ok so we open up source image 2 (new face).
We need to sort out levels initially so go image>adjustments>levels.
Set the input levels to: 14, 1.00, 80. Ok.
Now we create a new layer (layer>new>layer) and fill it wth colour #d3e6f5. Following this set the layer's blending mode to 'colour'.
You should currently have this:
Phase 2
The blotches of the skin are now quite obvious, so we need to smooth it out a bit.
Duplicate your layer with the face on (2nd layer up)
Go filter>noise>dust & scratches. Set radius to 6 and threshold to 0. Ok.
Go filter>blur>gaussian blur. Set radius to 3.2. Ok.
Then add a layer mask (layer palette - 'add layer mask') and fill it with black. This should make the blurry image disappear.
Now select the paintbrush tool (B), change hardness to 0 then press 'x' to switch your colour to white.
What you need to do now is paint a clean complexion on the face, without going over any details. Basically paint up to the edges of the face and lips and eyes without going over them. Don't worry about the nose - that practically has to vanish anyway.
It should now look like this:
Phase 3
Now you've smoothed out the skin, you need to highlight the eyes, eyebrows, lips and add some dimension to the face.
Create a new layer over the top of everything (create a new layer and drag it to the top in the layers palette).
Set the blending mode to 'overlay'
Select the paintbrush tool and change it's opacity to 30%. You now need to paint over the areas you want to enhance (the eyes, lips and hairline). Alter the opacity and the size of the paintbrush as you see fit and use the eraser at different opacity levels to lighten the effect.
When you're done it should look like this:
Phase 4
We now need to select our modified face and paste it over our lady from the Evanescence cover.
Select the magic wand tool (W), set the tolerance to 3 and click the white area surrounding our face. Press CTRL+SHIFT+I to invert the selection. Then copy your selection (CTRL+C)
Open up the first source image of the Evanescance cover and paste your clipboard in (CTRL+V)
Align the eyes up from both the layers by changing the new head layer's opacity to 50% and aligning the eyes manually.
Create a new layer mask (layer palette - new layer mask). Select the mask and then the colour black and with a large brush with hardness=0 go aroun the complete edge of the image.
This will nicely blend the new face onto the old one. Resulting in the image below:
Now if you spend a bit more time on it tweaking colours with adjustment layers and fiddle with the jawline you can create this:
