Why "Sample Template" as website name in search?

Can someone please tell me why "Sample Template" comes up as the website name when searching? I've changed all of my title tags to have the website name, but somewhere Google and Yahoo are pulling up Sample Template as the website name instead of the name given in the title tags.

Hi
I guess you use a template for your website,
without updating of that template , and this title is the title of the template
